SHIFT Kids Volunteer Application

Description/Expectations:
As a volunteer, you are committing to be downstairs in our children's ministry area, before, during, and for up to 30 min after our Sunday morning gatherings, loving our parents be allowing them to participate in worship upstairs. You will be expected to join with us at 30 min before service start for prayer with all of our workers and volunteers on before we each go to our area of serving. We have a children's curriculum that we expect the kids volunteers to teach, including - but not limited to - activities, crafts, and/or handouts involved in the curriculum. We have pre-packaged snacks available for children. It is the volunteer’s prerogative to serve these snacks, but also the responsibility of the volunteer to notice and know of any allergies each child has before serving snacks (as per allergy information on the child's name tag). At the end of each event, we expect toys and supplies to be sanitized, put away, and organized in preparation for the next event.

Security:
Each child will be given a sticker to wear with their name, family ID, and any allergies or special needs that child may have. Parents must present the corresponding ticket with matching information in order to pick up their kid(s). We expect each parent to come pick up their kid(s). Kids are NOT allowed to leave the room to wander upstairs to find their parent(s).

I hereby authorize SHIFT church and its designated agents and representatives to conduct a comprehensive review of my background causing a consumer report and/or an investigative consumer report to be generated for employment and/or volunteer purposes. I understand the scope of the consumer report/investigative consumer report may include, but is not limited to the following areas: verification of social security number; credit reports; current and previous residences; employment history, education background, character references; drug testing, civil and criminal history records from any criminal justice agency in any of all federal, state, county jurisdictions; driving records, birth records, and any other public records. I further authorize any individual, company, firm, corporation, or public agency to divulge any and all information, verbal or written, pertaining to me, to SHIFT church or its agents. I further authorize the complete release of any records or data received from other sources. SHIFT church and its designated agents and representatives shall maintain all information received from its authorization in a confidential manner in order to protect the applicants personal information, including, but not limited to, addresses, social security numbers, and dates of birth. Furthermore, I accept the terms presented to work in this role of Older Children's Worker, and commit to be on time and stay until each child has been picked up. I agree to follow the standards and procedures indicated in this commitment form in order to support the needs of SHIFT church during this season.
